Clinical and non-clinical Review management
Errors in complex systems are inevitable. Sadly errors in leading, managing and reviewing incidents often compound problems, lead to further patient and staff traumas. This can be both directly and (often more significantly) indirectly extremely costly to organisations, and more importantly staff and patients.
